Problem registering application

(I've already registered one, but I'm trying to register
another variant of the same app for test purposes - OAuth
frameworks seem a bit limited in support for multiple
endpoints for different versions of a single app - live, staging,
testing etc).
The problem I'm having is the following:
I fill out all of the fields in the form and click 'Create' but
the form is not submitted.
It looks like the JS is doing client side form validation and
is finding some problem with the data I've entered on the
form. The cursor is moved to the 'Application name' field
of the form, but there is no text anywhere to tell me what is
wrong. I've tried changing the text in this field to remove space,
different string lengths, etc but that does not seem to be the
problem; I've also tried using different browsers to see if that
might be the issue (Chrome, FF, Safari) - again no joy.
Anyone else seen this problem? Any pointers?
